Renewal Request Received

Thank You. We have successfully received your request for renewal. Please now allow 3 business days for the Physician to review the questionnaire answers you've provided. You will then receive a an email that contains both your renewed Medical approval, known as a Physicians Certification Form (PCF) along with instructions how to now submit that medical approval to the state.

Your renewal is not complete until the state receives the medical approval we provided, so please don't delay submitting that once it's received.  The state of Missouri will charge you a renewal application fee of $26.83.
